Short answer: Every path here supports the same destination: healthy NAD+ levels. Precursors (NMN, NR, niacin, niacinamide) are small molecules that survive digestion and carry the strongest human evidence [published human data]. Direct NAD+ skips every conversion step, delivered sublingually to bypass the gut it cannot survive.

This is the hub question of the whole NAD+ topic, and it deserves a full map rather than a two-horse race. We resell a direct-NAD+ sublingual strip — a bias we disclose up front — so we've made the precursor sections good enough that a precursor brand could publish them. Start with the NAD+ guide for compound background.

The size gradient is the whole story

Whether an oral NAD+ strategy works depends first on molecular size and charge. From largest to smallest:

MoleculeApprox. sizeGut-survival profile
NAD+~663 Da, chargedPoor — does not survive digestion intact
NMN~334 DaOrally bioavailable
NR~255–290 DaOrally bioavailable
NAR / NRH~255–256 DaResearch-stage
Niacin~123 DaWell absorbed
Niacinamide~122 DaWell absorbed

Smaller, uncharged molecules cross the gut wall; large, charged ones don't. That's membrane physics, not marketing.

The fork that created this market

Each strategy solves half the problem elegantly:

  • Direct NAD+ is the molecule you actually want — no conversion, no intermediate steps. Swallowed, it doesn't survive the gut; so direct delivery uses IV (clinic prices, chair time — see sublingual NAD+ vs IV) or the under-tongue mucosa, which is what strips were engineered for.
  • Precursors survive the gut beautifully and your cells convert them — a proven, practical route with one extra step between swallow and result.

Sublingual strips are the attempt to cut the knot: direct molecule, no needle, no clinic. Whether they deliver enough is the honest frontier, covered in does sublingual NAD+ work.

The precursor evidence — genuinely strong

Credit where due: the oral precursors hold the best human data in the category.

  • NR: Trammell et al., Nature Communications 2016 — oral NR is bioavailable and raises blood NAD+ in humans [published human data].
  • NMN: multiple randomized controlled trials at studied doses of 250–900 mg/day show raised blood NAD+ in humans [published human data]. (Studied ranges, not dosing advice.)
  • Niacin / niacinamide: the original NAD+ supporters — decades of use, well absorbed.

One shared caveat, stated once: "raises the NAD+ biomarker" is established; "produces a specific health outcome in healthy adults" is not — for any of them. And no head-to-head human trial ranks the precursors; see NMN vs NR and NMN vs NAD+.

The direct-route evidence — early and pointing the right way

  • Rat pharmacokinetics (iX Biopharma, 2024): roughly 2-fold plasma NAD+ peak within 10 minutes of sublingual dosing [preclinical evidence — company-sponsored].
  • Human pilot (iX Biopharma, 2024): open-label, company-sponsored, nine healthy participants; average whole-blood NAD+ up +59% at two weeks and +76% at six weeks [unpublished company pilot — not peer reviewed].

The regulatory tailwind (as of August 2026)

NMN's status has moved in consumers' favor: FDA exclusion on November 4, 2022, reversal on September 29, 2025, NDI acknowledgment reinstated December 2, 2025 — eligibility restored after active re-examination [verified regulatory record, retrieved 2026-08-16]. Full timeline at is NMN legal. NR, niacin, niacinamide, and NAD+ itself never went through that exclusion.

The honesty passage

The full disclosure, once: we sell the direct route, and its human evidence is a nine-person uncontrolled pilot — early, promising, and not proof. If your rule is "strongest published human data," the precursors win today, and we don't sell them. What we offer is the direct molecule for people choosing that route with open eyes, labeled as exactly what it is.

So which route fits your goal?

  • Evidence-maximalist, happy with capsules: a precursor (NMN or NR). Strong RCT record.
  • Direct molecule, needle-free, five-second daily habit: the NAD+ strips — $118 per 30-strip box.
  • Direct molecule, deepest track record, budget no object: IV NAD+.
  • Deciding between precursors: NMN vs NR walks that specific fork.

The bottom line

The NAD+ category is one of the few in wellness where every serious option rests on real, published science — precursors with their trials, direct NAD+ with its physics and fast-maturing early data. If the direct route fits how you live — no pills to swallow, no clinic to book, one strip and done — the NAD+ sublingual strips at $118 per 30-strip box are the cleanest daily expression of it. Choose the route you'll actually stick with; consistency is the real active ingredient.

Questions

NAD+ vs precursors — quick answers

Why can't you just swallow NAD+?

Intact NAD+ (~663 Da, charged) is broken down during digestion before it can be absorbed intact — which is why direct-NAD+ strategies use IV or mucosal routes that bypass the gut.

Which NAD+ precursor has the best evidence?

NR and NMN both have human trials showing raised blood NAD+ [published human data]; niacin and niacinamide have the longest use history. No head-to-head human trial ranks them.

Is NMN legal again?

As of August 2026: yes — FDA reversed its 2022 exclusion on September 29, 2025 and reinstated NMN's NDI acknowledgment on December 2, 2025. Restored eligibility, not deregulation. Our is NMN legal page tracks changes.

Is sublingual NAD+ proven to work?

Not yet proven — the evidence is a rat PK study and a nine-person uncontrolled company-sponsored pilot [preclinical + unpublished pilot]. Promising, early, and labeled that way on the product page.
